TRAFFIC LIGHTS SYSTEM - updated 25 January 2022

We are offering all services either by telephone consult or in person. Booked appointments only in Traffic Light Red - no walk-in or drop-in appointments.

The service may be slightly different than you are used to.

  • Our staff will help you make the right appointment.
  • If you are having a face-to-face appointment, you will be COVID-screened before your appointment and when you arrive at the clinic.
  • You will need to wear a mask when coming to the clinic and a medical mask during your appointment. We’ll give you a medical mask to wear.
  • If you require a support person with you, please let us know at the time of booking your appointment. Support people will be required to show their My Vaccine Pass either digitally or as a hardcopy at the time of your appointment.

New clients

Everyone is welcome at Family Planning. If you are a new client you will need to give us some details and we will give you a pamphlet about your rights as a Family Planning client.

Information about you - Client registration form

When you visit a clinic for the first time, you will be given a Client Registration form to fill in.

The information on this form is about you – your name, how we can contact you, your ethnicity and age.

You can fill in the form before your appointment and bring it with you if you'd like.
