New clients at our clinics need to fill in a form and will be given a pamphlet which details your rights as a Family Planning client.
Information about you - Client registration form
When you visit one of our clinics for the first time, you will be asked to fill in a Client Registration form.
The information on this form is about you – your name, how we can contact you, your ethnicity and age.
Fill the form in before your appointment and bring it with you to save time at the clinic.
