Notice to Family Planning Clients - Destruction of Medical Records

 

The medical records of Family Planning clients who have not been seen for at least 15 years at this Auckland Family Planning clinic, are currently in storage and will be destroyed at the end of May 2010:

Henderson Clinic: last seen between 1991-1993

If you know anyone who last attended this Family Planning clinic during these years, and who may wish to have their medical records, please ask them to contact before 31 May 2010:
